Probiotics Hold Potential as a Treatment for Patients with Irritable Bowel Syndrome
In a systematic review and meta-analysis of randomized, controlled, blinded trials investigating the effects of probiotics in the treatment of patients with irritable bowel syndrome (IBS), the authors conclude that probiotics may improve symptoms of IBS. The authors included results from 20 trials with a total of 1,404 subjects in their analysis. Compared to placebo, pooled analysis found probiotic use to be associated with improvement in global IBS symptoms (pooled relative risk: 0.77) and reduction in abdominal pain (pooled RR=0.78). Given the fact that, according to the authors, IBS affects between 3-25% of the general population, the results of this meta-analysis offer hope that probiotics may be a safe and effective tool in the treatment of patients with IBS. However, the authors caution, "While our analyses suggest that probiotic use may be associated with improvement in IBS symptoms compared to placebo, these results should be interpreted with caution, given the methodological limitations of contributing studies." Additional research is warranted.

Probiotics Protect Against Various Diarrhoeal Illnesses
In a review of studies related to the use of probiotics for the treatment of various gastrointestinal conditions, it was found that probiotics are effective in preventing and ameliorating various diarrhoeal illnesses in children and adults. Specifically, probiotics have been found to exert a protective effect against diarrhoeal illnesses including antibiotic-associated diarrhea in children and adults, and necrotizing enterocolitis in neonates. In order to enhance the anti-inflammatory effects of probiotics and restore colonization resistance of the commensal flora, current research is looking at the effects of combining probiotics with prebiotics. In addition, the use of new technologies is allowing for the elucidation of complex effects on gene expression in the gut, the probiotic, and bacterial pathogens. Finally, in order to treat gastrointestinal infection, novel therapeutic agents involving recombinant strains capable of binding bacterial toxins are being developed. This review suggests that probiotics have a very important role to play in preventing and helping to treat diarrhea/diarrhoeal illness. Further studies investigating strain-specific effects, dosing regimens, and contraindications are warranted.
