This study evaluated 20 patients admitted to a hospital on total parenteral nutrition who were given a parenteral nutrition enriched with glycyl-L-glutamine at 0.23 gm of glutamine per kg body weight, or a standard parenteral formula. After 2 weeks, the glutamine group's intestinal permeability was unchanged, whereas the permeability in the standard parenteral nutrition group was increased. Villous height remained the same in the glutamine group, but decreased in the standard group. The authors conclude the addition of glutamine to parenteral nutrition reduces the deterioration of gut permeability and maintains mucosal structure.

This article reviews the role and importance of total parenteral nutrition (TPN) as an adjunctive treatment to cancer. The article notes that it is important whenever possible to consume nutrients through the gastrointestinal tract since TPN can result in villous atrophy and a breakdown of the gut mucosal barrier, promoting translocation of gut bacteria into the blood stream and lymph nodes. One nutrient that is of extreme importance and is lacked in TPN is L-glutamine. Glutamine may be second only to glucose in support as a fuel for the body. It is the principal fuel for the intestinal mucosa. The body makes glutamine slowly. It is an essential amino acid for sick people. Blood and tissue glutamine concentrations can decline dramatically during catabolic states including cancer. Glutamine in ill patients becomes extremely low. Glutamine is not yet available for parenteral nutrition because it is unstable in solution. Glutamine can be ingested through the gut or by intravenous administration. When the gut mucosa is damaged and the mucosal barrier break down, the gut becomes more permeable and allows bacteria access to the lymph nodes and the bloodstream. This can result in multiple organ failure, villous atrophy and mucosal breakdown. Glutamine can also be taken up by the tumor, and there is a risk that the amino acid may stimulate tumor growth. Animal studies have shown that the malignant tumor competes with GI cells for glutamine. Based on animal models, the answer seems to be that exogenous glutamine administration helps the host by replenishing the muscle cell pool. Glutathione is another nutrient which is a free radical scavenger. It is important in cancer patients. Glutathione helps to protect the gut mucosa because it scavenges chemotherapeutic agents such as methotrexate, that damage the gastrointestinal tract. TPN is helpful because it replenishes gut glutathione levels without the effect on tumor uptake.
In conclusion, total parenteral nutrition and aggressive enteral care are a necessary part of the therapeutic regimen of oncologists. Well nourished cancer patients are better able to withstand the treatment and fight both infectious and noninfectious complications.
