This article reviews the role of vitamin A in immune function and in infection. Recent research of retinoic acid and retinoid X receptors provides a molecular basis for the action of vitamin A and its metabolites at the level of gene activation. Over a dozen trials showed that vitamin A supplementation reduced morbidity and mortality from infectious disease among children who have acute measles or who were from areas in which vitamin A deficiency is endemic. Vitamin A deficiency results in widespread changes in immunity, alterations in mucosal surfaces, impaired antibody responses to challenge with protein antigens, changes in lymphocyte subpopulations, and altered T- and B-cell function. Vitamin A and its metabolites are immune enhancers that have been shown to potentiate antibody responses to T cell-dependent antigens, increase lymphocyte proliferation and response to antigens and mitogens, inhibit apoptosis and restore mucosal cell surface integrity and function. Vitamin A and related retinoids may have potential applications in the treatment of infectious disease. IgG antibody responses to tetanus toxoid in children have been enhanced with high doses of vitamin A at 200,000 I.U. An estimated 124 million children are deficient in vitamin A and have a higher risk of death from infectious diseases. High dose vitamin A supplementation in children with acute measles is believed to contribute to a reduction in mortality. The possibility of vitamin A reducing the morbidity and mortality from which effects more than 300 million people worldwide are from tuberculosis which effects approximately ? of the world's population is unknown. It is estimated that 38 to 108 million individuals will be infected with HIV by the year 2,000. It is known that vitamin A deficiency is common among HIV patients.
